Sign-in Questionnaire for COVID-19

We ask that you answer the questions honestly, using the most current information you have.  Please submit this check-in questionnaire no more than one hour before the scheduled time of your appointment. This is for the safety of everyone that comes to training, and the sign-in questionnaire must be completed each time you come to a training session.  All responses are confidential and will only be shared with trainers on a need to know basis.
